Please report a communicable disease (flu, strep throat, pink eye, COVID-19, etc) to your building school nurse within 24 hours.
Parents will be contacted when students need to go home from school due to illness or injury.
OCSD is a food allergy aware district. Your building school nurse will work with parents, students, and the child’s allergist to reduce the risk of accidental exposure to foods in the school setting and to provide a safe educational environment for food-allergic students.
It is strongly encouraged that parents schedule, to the extent possible, a student’s medication administration outside of school hours. If medication, including over the counter medication, must be administered during school hours, please review the medication guidelines and complete the Medication Administration Form with a licensed prescriber.
Please use Final Forms to complete and update your child’s health information. A copy of your child’s current immunization record is required per ORC 3313.67 and can be uploaded into Final Forms.
